Commonly Used Forms at Live Oak

For your convenience, we are now offering the ability to make your pledge payments and other contribution via electronic transfer directly from your bank account. Please print out and complete this authorization form, attach a voided check or savings deposit slip and mail it to the church (or put it in the assistant treasurer's mailbox in the office).
